Summary Care Record - Opt Out FormWhat is a Summary Care Record (SCR)?

Summary Care Records consist of summary patient information, - such as medication, adverse reactions, allergies and personal details (eg: your name, address, age and NHS number), - which can be downloaded from GPs records.

It is intended that this information will be shared electronically with and used by emergency and unscheduled care services, when other records are unavailable. Patients will also be able to view their summary care record online through the Healthspace website.

The view of the BMA & many GPs

The British Medical Association (BMA) has stated that patients have not been given adequate information about Summary Care Records.

Under the current 'consent to view' mode, patients will automatically have a Summary Care Record created unless they choose to opt-out.

John May (BMA's patient liaison group) says:

"There needs to be a higher profile national information campaign to ensure everyone can make an informed choice about whether or not they want to be included,"

"We also think it is important that opting-out is made easier. At the moment there's no opt-out form in the patient information packs being sent to patients across the country. They either have to take the time out of their day to go and see their GP, or phone a call centre, or download a form from the internet and post it in."

A report published in 2008 showed that of 614,052 participants in a pilot scheme, only 0.8% declined or requested restricted access to their record; however, a focus group of 103 patients found one-third were unaware of the existence of the scheme.

Anyone who DOES NOT wish to have a Summary Care Record will need to complete an opt out form and give this to their GP by 2nd July 2010.
